Abstrak

Medication error merupakan kejadian yang dapat merugikan pasien, namun sebenarnya dapat dicegah. Salah satu faktor yang memicu terjadinya medication error adalah ketidak lengkapan resep. Penelitian ini bertujuan untuk mengevaluasi kelengkapan resep terhadap potensi terjadinya medication error di RS Madina Bukittinggi tahun 2023. Penelitian menggunakan desain deskriptif kualitatif dengan pengambilan data retrospektif. Sampel sebanyak 398 lembar resep diperoleh melalui metode porposional sampling dengan rumus Slovin. Instrumen penelitian berupa checklist skrining resep berdasarkan Permenkes No.72 Tahun 2016. Hasil penelitian menunjukkan bahwa aspek administratif kelengkapan resep menunjukkan 45,45% lengkap, sementara ketidak lengkapan mencapai 54,55%. Pada aspek farmasetik, kelengkapan resep sebesar 84,7%, sedangkan ketidak lengkapan 15,3%. Aspek klinis memiliki lengkap/aman 83,75% dan tidak lengkap/berpoteni error 16,25%. Hasil penelitian menunjukkan bahwa identifikasi medication error berdasarkan klasifikasi NCC MERP menunjukan kategori A tercatat 0,08%, kategori B sebesar 96,05%, kategori C 1,72%, kategori D 1,64%, dan kategori E 0,51%. Dapat disimpulkan bahwa seluruh resep yang dianalisis belum sepenuhnya memenuhi kelengkapan sesuai standar Permenkes No.72 Tahun 2016, sehingga masih berpotensi menimbulkan medication error. Saran dari penelitian ini adalah perlunya peningkatan ketelitian dokter dalam menuliskan resep, penerapan sistem elektronik untuk mendukung kelengkapan resep, serta penguatan peran apoteker dalam melakukan skrining resep secara konsisten guna meminimalkan risiko medication error dan meningkatkan keselamatan pasien. Abstract (EN): Medication errors are incidents that can harm patients but are, in fact, preventable. One of the factors that can trigger medication errors is incomplete prescriptions. This study aims to evaluate the completeness of prescriptions and their potential to cause medication errors at Madina Hospital Bukittinggi in 2023. The study used a descriptive qualitative design with retrospective data collection. A total of 398 prescription sheets were obtained using proportional sampling based on Slovin's formula. The research instrument was a prescription screening checklist based on Ministry of Health Regulation No. 72 of 2016. The results showed that for the administrative aspect, 45.45% of prescriptions were complete, while 54.55% were incomplete. For the pharmaceutical aspect, 84.7% were complete and 15.3% were incomplete. In the clinical aspect, 83.75% were complete/safe, while 16.25% were incomplete/at risk of error. Based on NCC MERP classification, the identification of medication errors showed: Category A: 0.08% Category B: 96.05% Category C: 1.72% Category D: 1.64% Category E: 0.51% It can be concluded that the prescriptions analyzed did not fully meet the completeness standards set by Ministry of Health Regulation No. 72 of 2016 and still pose a potential risk for medication errors. This study recommends increasing physicians’ accuracy in writing prescriptions, implementing electronic systems to support prescription completeness, and strengthening the role of pharmacists in consistently screening prescriptions to minimize medication error risks and enhance patient safety.
