Abstract (EN): Drug Related Problems (DRPs) are conditions associated with drug therapy that can potentially interfere with the achievement of desired clinical outcomes. Kidney failure is a disorder in kidney function that causes the blood filtration process to not work properly, resulting in the accumulation of waste, fluids, and electrolytes in the body. Kidney failure may occur suddenly (acute) or develop gradually (chronic). Therapeutic failure refers to the condition in which the desired therapeutic effect is not achieved. This study aimed to determine the effect of DRPs on the therapeutic success of chronic kidney disease (CKD) patients with comorbidities in the outpatient department of Achmad Mochtar Hospital Bukittinggi in 2023. This research was a descriptive quantitative study with an analytic design using the chi-square test. Data were collected from medical records and prescription sheets. The study population consisted of 10,270 patients, and a total of 100 respondents were selected using the simple random sampling method based on the Slovin formula. The results showed that most respondents were male (51%), in the late elderly age group (36%), had a low educational background (55%), and were employed (62%). Identification of DRPs revealed no untreated indications (74%), no inappropriate drug selection (84%), subtherapeutic dosing (93%), no overdose (87%), drug interactions (74%), and no unnecessary drug use (85%). Therapeutic success was achieved in 67% of patients. Statistical analysis showed no significant relationship between untreated indications (p=0.175), inappropriate drug selection (p=0.871), subtherapeutic dose (p=0.321), overdose (p=0.250), or drug interactions (p=0.839) and therapeutic success. However, there was a significant relationship between unnecessary drug use and therapeutic success (p=0.046; OR=4.95).
