Abstrak
Latar Belakang: Case-mix merupakan sistem pembayaran kesehatan yang berhubungan dengan mutu,pemerataan, dan keterjangkauan pelayanan kesehatan. Permasalahannya masih tinggi biaya pelayanankesehatan, sehingga pemerintah melalui program Case-mix untukmenetapkan standarisasi biaya pelayanan kesehatan pada pasien rawat inap. Penerapannya sudah dilaksanakan pada RSUD Sijunjung sejak tahun 2014, permasalahan dalam pelaksanaannya adalah kualitas informasi yang tidak efektif, diantaranya kelengkapan dan kejelasan penulisan diagnosis serta ketepatan kode.Tujuan dari penelitian ini adalah untuk mengetahui kebijakan untuk mengisi diagnosa utama dan kode diagnosa akurat pada rekam medis. Metode: Jenis penelitian adalah kualitatif, informan yang didapatselama penelitian melalui wawancara mendalam adalah 10 informan. Hasil:Diketahui bahwa kebijakanCase-mix Ina-CBGs sudah ada, namunSOP dan petunjuk teknis dalam pengisian rekam medis secara operasional belumdisosialisasikan, Tim Case-mix belum dibentuk, motivasi dan edukasi belum optimal, monitoring/evaluasi belum diterapkan.Hasil triangulasi teridentifikasi beberapa permasalahan untuk mengisi diagnosa utama dan kode diagnosatidak akurat, karena SIMRS belum diterapkan menyebabkan data tidak sinkron yang berdampak pada biaya pelayanan. Kesimpulan: Kebijakan sangat penting dalam mengelola aktivitas dan komunikasi antara staf,karena sangat mendukung hasil dari implementasi kebijakan. Disamping itu didalam pengisian rekam medis (penulisan diagnosis) sebaiknya legible (mudah terbaca), sehingga memudahkan kegiatan pengkodean, karena dampaknyasangat besar pada biaya pelayanan kesehatan.Saran:Perlu koordinasi antara manajemen rumah sakit, tim medis dengan Tim pelaksana BPJS tentang petunjuk pelaksana dan petunjung teknis.Oleh sebab itu, diharapkan kepada manajemen rumah sakit untuk menetapkan Standar Operasional Prosedur,menggunakanSIMRS dengan database yang tepat dan sesuai, membentuk Tim Case-mix, melakukan sosialisasi, motivasi dan edukasi, serta monitoring dan evaluasidalam pelaksanaan Case-mix Ina-CBGs. Abstract (EN): Background: Case-mix is a health payment system which is related to the quality, equity and affordability of health care. The problem is the high cost of health services, so that the governments through the Case-mix program to establish the standardization of health care costs for the inpatient care. This program has been implemented in Sijunjung local hospital since 2014; however the quality of the information was not effective, such as the comprehensiveness and the clarity of writing diagnosis and the accuracy of the code. The aim of this study is to determine the policy to fill the main diagnosis and the accurate diagnosis code in the medical record. Methods: The study is a qualitative method; informants obtained along the research through in-depth interviews were 10 informants. Results: The Case-mix Ina-CBGs policy has already existed, but the SOP and the technical guidance in filling out medical records operationally have not been socialized, The Case-mix team has not been established, the motivation and the education were not optimal, monitoring / evaluation has not been applied. The triangulation results identified that there are some problems to fill the main diagnosis and the diagnosis code are not accurate, because SIMRS has not been applied thus it causes the asynchronous data which is affected to the cost of the health services. Conclusions: Policy is a very essential part in managing the activities and the communication among the staff, because it supports the results of the implementation. Besides, in filling out medical record (writing diagnosis), it should be legible (easily legible), thus making it easier in coding activities, because it has a significant effect towards the health care cost. Suggestion: coordination among hospital managements is necessary; the medical team with the BPJS staff should coordinate on the implementation and the technical guidelines. Therefore, it is expected to the hospital management to establish the Standard Operating Procedures, using SIMRS with the right and appropriate database, to form the Case-mix team, socialization, motivation, education, monitoring and evaluation in the implementation of the Case-mix InaCBGs.
