Abstrak
WHO menyatakan hipertensi di dunia 1,28 miliar dan diabetes di Indonesia 8,4 juta. Puskesmas IV Koto melaksanakan Program Senapan Kaliber. Tujuan penelitian analisis faktor yang berhubungan dengan pemanfaatan Senapan Kaliber prolanis Puskesmas IV Koto Kabupaten Agam Tahun 2025. Penelitian kuantitatif dengan kuisioner, pengambilan sampel proportional simple random sampling: hipertensi 173 dan diabetes 37 orang, data kualitiatif dengan wawancara, dilakukan April sampai Juni 2025. Variabel kuantitatif: pengetahuan, sikap, tindakan, dukungan keluarga dan peran nakes terhadap Senapan Kaliber. Analisis univariat, bivariat dan multivariat, dan triangulasi data untuk penelitian kualitatif. Hasil kuantitatif: hipertensi, univariat: variabel pengetahuan rendah (56,1%), sikap rendah (62,4%) tindakan rendah (52,%), dukungan keluarga rendah (54,3%) dan peran nakes tinggi (64,2%), tidak memanfaatkan Senapan Kaliber (59%). Bivariat: variabel yang tidak berhubungan, pengetahuan, sikap, dukungan keluarga dan peran nakes dengan pemanfaatan Senapan Kaliber (p value >α), variabel berhubungan tindakan dengan p- value=0,001. Multivariat: secara bersama-sama variabel berpengaruh adalah sikap, dukungan keluarga dan tindakan, yang paling berpengaruh adalah tindakan OR 3,107. Diabetes: univariat: pengetahuan tinggi (70,3%), sikap tinggi (75,7%), tindakan rendah (67,6%), dukungan keluarga rendah (56,8%), peran nakes tinggi (75,7%), tidak memanfaatkan Senapan Kaliber (70,3%). Bivariat: variabel pengetahuan terdapat hubungan p value 0,05, sikap, tindakan, dukungan keluarga, peran nakes, tidak ada hubungan dengan pemanfaatan Senapan Kaliber (p value >α). Hasil kualitatif: SDM mencukupi, dana mencukupi, sarana, prasarana belum mencukupi, kebijakan ada, proses sesuai manajemen, output belum tercapai. Kesimpulan pemanfaatan Senapan Kaliber rendah dan output tidak tercapai, dipengaruhi pengetahuan, sikap dan tindakan serta dukungan keluarga, yang paling berpengaruh adalah tindakan. Saran Puskesmas meningkatkan upaya untuk pengembangan Inovasi Senapan Kaliber on The dor (Senam Prolanis Amankan Kartu Kendali berobat melalui On Call, Therapy, Edukasi, Dukungan Keluarga, Home Sharing atau kunjungan rumah). Abstract (EN): The World Health Organization (WHO) reports that globally, 1.28 billion people suffer from hypertension, and in Indonesia, 8.4 million people live with diabetes. Puskesmas IV Koto has implemented an innovative program called Senapan Kaliber Prolanis Exercise to Secure the Medical Control Card. This study aims to analyze the factors associated with the utilization of the Senapan Kaliber Prolanis program at Puskesmas IV Koto, Agam District, in 2025. This was a quantitative study using a questionnaire, with proportional simple random sampling: 173 respondents with hypertension and 37 with diabetes. Qualitative data were also collected through interviews. The study was conducted from April to June 2025. The quantitative variables included knowledge, attitude, practice, family support, and the role of healthcare workers regarding the Senapan Kaliber program. Data were analyzed using univariate, bivariate, and multivariate analysis, and data triangulation was applied for the qualitative aspect. Quantitative Results For hypertension patients Univariate results showed low levels of knowledge (56.1%), attitude (62.4%), practice (52%), and family support (54.3%), while the role of healthcare workers was high (64.2%). A total of 59% did not utilize the Senapan Kaliber program. Bivariate analysis showed no significant relationship between knowledge, attitude, family support, and healthcare workers’ role with program utilization (p > α), but there was a significant relationship with practice (p = 0.001). Multivariate analysis showed that attitude, family support, and practice together influenced utilization, with practice being the most influential factor (OR = 3.107). For diabetes patients: Univariate results showed high levels of knowledge (70.3%) and attitude (75.7%), but low practice (67.6%) and family support (56.8%), with a high role of healthcare workers (75.7%). However, 70.3% still did not utilize the Senapan Kaliber program. Bivariate analysis found that only knowledge was significantly associated (p = 0.05), while attitude, practice, family support, and healthcare workers’ role were not (p > α). Qualitative Results: Human resources and funding were adequate, but infrastructure and facilities were insufficient. Policies were in place, and implementation followed standard management processes, but the expected outcomes had not been achieved. Conclusion: The utilization of the Senapan Kaliber program was low, and program outcomes had not been achieved. The influencing factors included knowledge, attitude, practice, and family support, with practice being the most influential. Recommendation Community health center should enhance efforts to improve the implementation and utilization of the Senapan Kaliber on The dor (Prolanis Exercise to Secure the Medical Control Card through On Call, Therapy, Education, Family Support, Home Sharing or home visits)program.
